PDPM and PDGM have made therapy utilization sparse compared to what was able to be offered over the past 20+ years in the SNF and home health sector respectively. When therapy was the driving factor of payment models, there was ample, if not too much, therapy services resulting in good outcomes, but very high costs to CMS.
As payment models shift towards value-based care models, the onus is on the "payvider" to determine the best mix between outcomes and costs; to choose the most effective and efficient care providers. Since Evolution Rehab Group is a part B provider, the costs of therapy are truly at least 50% of not 75% less than SNF and Home Health models of care for the same utilization of therapy.
A new option of sending patients home with just outpatient in the home therapy can not only keep costs low, but can allow for daily therapy, where new home care models and even SNF models won't pay for such utilization. Thereby increasing outcomes, front-loading care if needed, and reducing costs.
Our Medicare Certified Rehab Agency predominately contracts with Medicare, but we can work within other payment models and even direct contracts for the cases of capitated, full risk, partial risk, or alternative models like direct contracting, single provider ACOs, ACOs, PACE, and others.
Average cost of 14 sessions of therapy + custodial and nursing care in a skilled rehab is about $8,000 for 20 days of care, for home health, utilization costs over 45 days would be about $4,000, and in our outpatient-at-home model, would be about $1450. We can get cases started same day or next day as needed for facility discharges, and can take direct referrals as needed with no Face to Face or other burdensome requirements.
All we need is a prescription faxed to use for PT,OT, or ST services, with the patient's basic demographics, and once the case is started, we will send a plan of care to be signed every 6 weeks if needed. Patients are entitled to services so long as care is skilled, reasonable, necessary, and restorative without limitation.
